If you are a patient that looks after someone who is ill, frail, disabled or mentally ill?  Whether they get the ‘Care Component’ to pay for a Carer, you are important to us.

Caring for someone is an important and valuable role in the community, which is often a 24-hour job that can be very demanding and isolating for the carer.

We understand that your role within the community is a vital one and recognise that it is important that you are looked after too.

Our friendly Social Prescribers can make sure that Carers receive appropriate support by way of access to accurate information on a range of topics such as entitlement to benefits, respite care and a listening ear when things get too much, they can help with arranging appliances around the home, befriending services, arranging transport to appointments, to name just a few.

We offer all our patients over the age of 75 a health check and encourage for their carers (if you have one) to also be present at the time of the call or the appointment. If this is unsuitable then we can arrange a different time/day to suit your needs.
